An outdated image surviving on a website after a redesign is usually not a mistake by any one person. On one release, the retail listing and the digital edition were both updated correctly, and the old cover ran on the website for weeks anyway.

What happened

A cover redesign went out. The person responsible for the retail listing updated it correctly, on time. The person responsible for the digital edition updated it correctly, on time. The website was nobody's named job, and the old cover stayed live there for weeks.

What it cost

A buyer saw one cover on the website, paid, and opened a delivery email with a different cover waiting for them. Both of the jobs that touched the release had been done properly. The gap sat between them, in a handoff nobody owned.

Why it failed

This is the quiet version of a release failure: it doesn't need anybody to be careless. It only needs two people to each do their own job correctly, on either side of a surface that neither of them was told belonged to them. A failure that needs carelessness is rare. A failure that only needs two correct jobs and an unowned gap between them happens constantly, in any release with more than one surface carrying the same asset.

What we changed

A written register now lists every surface a cover, price or title appears on: retail listing, delivery email, website, social preview, the file itself. One person is named to walk that register the same day anything on it changes, comparing every listed surface against the canonical file and fixing any mismatch before moving to the next task. The register costs nothing to start and almost everything to skip.

How to spot it in your own account

List every place your current cover, price, or title actually appears: store listing, website, email templates, social previews, the product file itself. Count them. Most products live in more places than anyone guesses before counting. If no single person is named to check every one of those places on the day something changes, you have an unowned gap waiting for its own version of this failure.
